The Sebastopol office, called the Eye Center, is located at the corner of Palm and Petaluma Avenues and it is next door to Palm Drive Hospital. Take Highway 12 from Santa Rosa, or Highway 116 to Sebastopol from Cotati/Rohnert Park.

This is the main Eye Center for Eye Associates of Sebastopol Medical Group where we have major eye care diagnostic and therapeutic equipment, numerous exam rooms, and our business office.

The building also houses our own state-of-the-art surgery center called the Sebastopol Ambulatory Surgery Center.
